Deliverable 3: Create a Survey

For this assignment, you are tasked with creating a short, self-administered survey. Your survey should be clear and understandable such that someone could pick up the survey and take it entirely on their own (e.g. without your help to guide them through the questions).Your survey should be 4 pages in length and have about 25 questions.

Survey Goals: Imagine you are designing a survey that will be administered to SDSU students about their perceptions of San Diego State University’s COVID-19 response. You want to know how students have experienced college this school year (e.g. what’s gone well, what hasn’t gone well, what adjustments they have had to make), whether they think the university has done enough to keep students safe, and what, if any, personal concerns they have about COVID, their schooling, work, etc. Imagine that your survey would be programmed online and a link to your survey would be emailed out to all SDSU students (undergraduate and graduate students) asking them to complete it.

Your survey should include:

  • An opening description of what the purpose of the survey is, why the respondent is being asked to take it, any benefit to the respondent for taking it, instructions they may need for how to take the survey (including how to return the survey), and an explanation as to the extent of confidentiality and anonymity that can be expected, etc.
  • Clear instructions for each section of the survey about how the respondent should answer the questions (e.g. “select the answer choice that best represents how you feel” or “rank the following from 1 to 10 where 1 represents your smallest need and 10 represents your biggest need” etc.)
  • Well-organized and clear section headings and clearly numbered questions to help guide the reader through the survey.
  • Demographic questions (e.g. sex, age, race/ethnicity, year in school, and/or other background questions about your participants that you think are relevant to the purpose of your survey).Remember that we don’t want to collect personal information about people that is not necessary for us to achieve our research goals.Be thoughtful about what demographic information is important to collect.
  • A combination of close-ended and open-ended questions to address your survey goals.Your survey should have a good mix of both types of questions.
